Falls among older adults pose a significant risk, leading to life-altering injuries and imposing substantial healthcare costs. Extensive research as identified numerous fall risk factors across physical, psychological, pharmacological, and environmental domains. This study aims to develop and assess the feasibility of a Fall Risk Identification and Management (FRIM) model for older Veterans in primary care settings. The model aims to integrate fall risk screening, telehealth-based assessments, and referrals to existing VHA interventions to prevent falls. The research includes a feasibility pilot study to refine and evaluate the model before conducting a larger pragmatic randomized controlled trial.
